Eyre and Far North Local Health Network seeks a Community Support Worker for a temporary part-time position. The role requires 22.5 hours of work per week up to July 2025.
Key Responsibilities include providing personal care, housekeeping, meal preparation, and social support to clients in line with their care plans.
Foster strong relationships with clients, their families, and other health professionals.
Monitor client well-being, report changes in health or functionality, and provide diversional therapy as needed.
Support clients in achieving their goals under the Consumer Directed Care model, promoting independence and choice.
Requirements
* Certificate 3 in Individual Support (Ageing, Disability, or Home and Community).
* Current driver's licence and access to a reliable, insured vehicle.
* Experience in the provision of direct care service in a health-related field and experience dealing with aged, frail, or disabled persons.
* Ability to work independently and as part of a team.
* Excellent communication skills and the ability to support people from diverse backgrounds.
* Flexibility, patience, and a customer-focused approach to care.
