We are seeking an experienced and dedicated Care Manager who is passionate about providing high-quality care to older people. The ideal candidate will have previous experience in a similar role and a strong understanding of the aged care industry and regulatory requirements. They will also possess excellent leadership and communication skills, and the ability to build effective relationships with residents, staff, and families.
About the role: As the Care Manager, you will have a critical role in ensuring that our residents receive the highest standard of care and feel secure in their environment. Your responsibilities will include:
Implementing and maintaining a person-centred model of care, promoting wellness and engagement for residentsEnsuring compliance with legislative requirements and the Aged Care Quality StandardsExperience in change management and ability to participate in the on-call rosterProviding professional leadership and management to clinical and allied health team membersDeveloping and implementing care plans and conducting assessments and clinical education | mentoringMonitoring and evaluating the quality of care providedEnsuring effective communication and collaboration with residents, families, and other stakeholdersContributing to the development and implementation of policies and proceduresAbout you: The ideal candidate will have:
