Lead Multidisciplinary Teams: Facilitate a collaborative approach to support residents’ ongoing, meaningful lives. Coordinate Care: Monitor and manage clinical and holistic care, ensuring best practices and effective care planning. Family Engagement: Maintain communication with families, addressing concerns and scheduling annual conferences. Medication Management: Oversee medication practices, ensuring compliance with training and regulations. Evaluate Care: Assess and document residents’ responses to care, updating plans as needed. Incident Reporting: Manage clinical incident reporting and implement improvement opportunities. Staff Supervision: Ensure staff work within their scope and support their development through education and performance reviews. Care Assessment: Lead care assessments and ensure funding claims are accurate and substantiated. AN-ACC Coordination: Work with the AN-ACC Coordinator to ensure accurate submissions and compliance. Budget Monitoring: Assist with budget planning and report discrepancies in financial reports. Recruitment & Retention: Support staff recruitment, retention, and performance management. Quality Assurance: Participate in quality activities and ensure compliance with Aged Care Standards. Professional Development: Maintain personal development and compliance with Baptistcare’s policies and Code of Conduct. Mentorship: Provide guidance and support for Care Partners and new staff, including probationary reviews and training. Skills and Experience: Tertiary qualifications in Nursing and current registration with AHPRA as a Registered Nurse (Minimum 3 years) Previous experience at a senior clinical care level working in aged care Demonstrated leadership and influencing skills Sound understanding of care assessment in the Aged Care Sector and use of the ANN-AC Demonstrated effective interpersonal and communication skills (written and verbal). Sound understanding of relevant legislation and experience in meeting Australian Aged Care Quality Standards and Outcomes Demonstrated understanding of the person-centred approach in aged care and a commitment to providing a customer centric approach Demonstrated proficiency in the use of a wide range of IT applications Experience working at advanced skill level in the field of geriatric medicine, chronic disease management, palliative care, dementia care, or another field relevant to the care persons over the age of 65 An ability to understand financial management principles and interpret financial reports An understanding of contemporary Aged Care issues and opportunities Willingness to undergo a pre-employment medical Willingness to obtain and NDIS clearance About Us BaptistCare is a leading for-purpose Christian-based care organisation that supports thousands of people across more than 100 locations in NSW, ACT and WA.
