About the Role:
As a Clinical Care Coordinator, you will play a crucial role in managing client care and well-being. Your responsibilities will include specialized areas such as continence management, conducting clinical assessments and interventions, medication management, dementia care support, diabetes management, wound management, and education. You will ensure the safe provision of clinical care, compliance with regulations, and build positive relationships with clients and stakeholders.
Key Responsibilities Include:
1. Conduct clinical assessments and interventions in specialized areas such as wound management, continence management, medication management, dementia care support, and diabetes management and education.
2. Manage a small case load
3. Initiate service agreements with external providers when necessary.
4. Demonstrate comprehensive knowledge of industry regulations and best practices.
5. Efficiently manage client care within Bodewell policies and procedures.
6. Mentor and support direct care staff.
7. Respond to clinical inquiries and work with health professionals to develop care plans.
8. Monitor and review care plans within budget limitations.
9. Advocate for consumers to ensure timely clinical care.
10. Coordinate Peer Review Meetings and follow-ups.
11. Network and build relationships with community resources.
12. Ensure compliance with legislation, guidelines, and standards.
13. Maintain consumer confidentiality and privacy.
14. Participate in quality reporting activities.

Qualifications:
