Overview
We are looking for an experienced Registered Nurse to join our Home Support Team as a Clinical Coordinator - Home Support who will provide services appropriate to the needs of the Home Support client/consumer.
In this role, you will apply clinical experience, specialist clinical knowledge and professional competence to plan, implement and evaluate a comprehensive and integrated range of services, provide clinical oversight for services to Home Support clients/consumers in the community setting and provide education and support to Community Support Workers to ensure they have the skills and competencies to undertake their role.
