Clinical Nurse (Medical Unit Discharge Planner) The Medical Unit Discharge Planner is a Clinical Nurse responsible for utilising advanced problem-solving, expert planning, and coordination skills to enhance patient flow throughout the Medical Unit. This role focuses on optimising clinical care to ensure patients experience a safe, timely, effective, and seamless discharge process. About the role: The Clinical Nurse functions at an advanced level, supporting the delivery of high-quality, evidence-based nursing care. This role plays a key part in mentoring and guiding other nursing staff while collaborating with the Nurse Unit Manager and the multidisciplinary team in managing and developing clinical services. Although primarily focused on clinical duties, the position also assists Nurse Grade 7 or higher with management activities. About you: Mandatory qualification/s: Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA). Demonstrated ability to effectively plan and coordinate patient flow movements throughout the entire Medical Unit experience. Knowledge of quality improvement processes, active participation in the identification, development, and implantation of patient flow opportunities to facilitate an improvement in flow through the Medical Unit.
