Our well-established Eden Private Hospital is a 48-bed rehabilitation/sub-acute medical hospital located in the idyllic Noosa Hinterland township of Cooroy. Eden Private Hospital is dedicated to excellence through the provision of compassionate quality care, teamwork, and innovation.
Expressions of interest are being sought from suitably qualified and experienced Social Workers to join our dynamic team of allied health professionals.
About the Role
Overall responsibility for patient care and input to the rehabilitation programs at Eden Private Hospital. These include (but are not limited to) orthopaedic, neurological, and other medical conditions.
The Social Worker’s primary focus is on the following:
* Provision of assessment, planning, and intervention for referrals (including supports for families/carers), with demonstrable attention to professional standards, knowledge, theory, and skills.
* Demonstrate a sound knowledge base related to social and health issues specific to effective linkages and referral processes made to relevant agencies and service providers.
* Assess, plan, implement, and evaluate Social Work care and service to achieve optimal patient outcomes.
* Develop and maintain best practices in Social Work and discharge planning, adopting a holistic approach.
* Provide short-term counselling and support services to patients and their families and carers.
* Link patients, their families, and carers into appropriate longer-term services or supports as required.
* Assist patients and their families and carers through provision of information, advice, advocacy, services, and appropriate referrals regarding legal, financial, housing issues, etc.
About You

* Experience in a similar role with experience in a hospital setting.
* Highly developed patient assessment and management skills.
* Strong work ethic and excellent leadership skills.
* Focus on service improvement for delivery of excellent patient care and positive patient outcomes.
* Effective time management and coordination skills.
* Demonstrated high standards of professional and ethical practice.
* Promotes safety, security, and personal integrity in the workplace.
* Ability to work with minimal supervision.
Mandatory Qualifications:
* Accreditation with the AASW.
* Clinical experience, including discharge planning, in medical and/or rehabilitation hospital settings.
