Are you ready to make a real difference in the lives of individuals facing severe and recurring mental illness? We have an incredible opportunity for you to join our passionate team at the Housing and Accommodation Support Initiative (HASI)!
The Housing and Accommodation Support Initiative (HASI) aims to support people living with a mental illness to maintain and enhance their housing, participate in community life, increase their functioning, and enhance their overall wellbeing.
In this role, you will provide ongoing case management and practical supports with clients to improve mental health outcomes. The team provides individualised support to move away from hospitalisation and towards recovery, community involvement, training, employment, and permanent housing options.
Your key responsibilities will be to:
* Respond to referrals of clients to the service from internal and external support services.
* Provide outreach services to clients – successfully inducting them into the program and creating individualised support plans.
* Develop and maintain relationships with internal and external stakeholders including service partners and government agencies.
* Administration - Complete a range of internal and external reports relating to clients and the program including risk assessments, individualised support plans and program promotion.
Requirements for Success:
* Relevant experience or qualifications in community services.
* Demonstrated experience of case management working with people with complex mental health issues.
* Skills in building rapport with clients while also liaising, negotiating, and advocating for consumers with a wide variety of stakeholders.
* Computer literacy preferably using Windows.
* First Aid certificate and Driver License.
* Willingness to undertake any relevant training opportunities for professional development relevant to the role.
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
