Provide a high quality, person-centred service to help promote independence for our clients living in the community. This role is diverse and dynamic, responsible for the assessment, management and treatment for clients referred with various health conditions. You will work with a supportive and motivated team that will ensure you are well prepared and supported for each client you see. Maintain accurate and up-to-date medical records and documentation. Adhere to all policies and procedures, as well as state and federal regulations. Work collaboratively with other healthcare professionals, including physicians, nurses, and support staff, to ensure quality patient care. Participate in ongoing education and training to maintain and enhance nursing skills and knowledge. Other Requirements: Strong critical thinking and problem-solving skills. Excellent communication and interpersonal skills.
