The Hospital In The Home (HITH) service specialises in providing care that avoids Emergency Department presentations, hospital avoidance and supports early discharge from hospital. Working in collaboration with the HITH multidisciplinary team, the HITH Registered Nurse also facilitates case management and care planning through the assessment, planning, implementation and evaluation of nursing care to keep patients safe in the community and out of hospital.
The HITH service works out of three different sites, and successful candidates will be expected to work at each site on a rotating roster - Long Jetty, Gosford and Wyong hospitals.
Working hours: (A) shift 07:00- 15:30hrs Monday- Sunday. (P) shift 12:00- 20:30hrs Monday- Sunday.
This is a maternity relief position and should the permanent incumbent require an early return from maternity leave, this contract of employment will cease with two weeks notice. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.
