We are currently seeking a Registered Nurse to join our Community Nursing team; this is a full-time position. About Us East Grampians Health Service is based in Ararat and Willaura (Regional Victoria) and takes its name from the magnificent mountain range that provides a backdrop to both townships. Community Nursing is a part of the broader Community Services Department located at the Ararat campus. We provide community nursing across the catchment area of Ararat Rural City, including Palliative Care, District Nursing, Hospital in the Home, Hospital Admission Risk Program (HARP), and specialist nursing programs. Each program area aims to provide awareness and education that enables community members to better understand their health issues and take greater responsibility for improving their health outcomes. About the Role District Nurses are independent practitioners who partner with consumers in shared decision-making processes to ensure consumers are partners in their own care. Responsibilities include assessment and care planning, facilitating and coordinating evidence-based interventions relevant to the client's condition(s), and liaison with the team and other services. About You Current registration with AHPRA as a Registered Nurse A demonstrated commitment to providing care that is client-centered, strengths-based, and underpinned by a re-enablement and restorative philosophy A current and valid Victorian driver's license Ability to contribute and practice collaboratively as part of a multidisciplinary team Appointment is subject to immunisation clearance and satisfactory Employment Checks (Police and Working With Children Checks).
