Job Description
We invite applications from Registered Nurses who would like to join our dynamic Community Health team in District Nursing in Mortlake and Terang.
The primary role of the District nurse is to provide person-centred care, focusing on maximising functional capacity, home-based acute care, improved self-management strategies and coordination of service delivery to clients in their own homes.
Key Responsibilities:
* Provide evidenced based practice
* Demonstrate an understanding of and commitment to nursing in the community setting and quality improvement
* Liaise with other care providers
A current driver's license is essential. Post-graduate qualifications are highly regarded.
