Our core business consists of the coordination and delivery of home support services and community care across Tasmania.
Job tasks and responsibilities
To provide support and assistance to consumers as a partner in the coordination and management of care and services within the Home Care Package Program (HCP).
* Coordinating all elements of the consumers’ Home Care Packages.
* Service intake, coordination of home care and support, consumer agreements, support planning, referral, and care-management, including but not limited to:
o Identification of consumer's needs and goals.
o Development of consumer directed goal-based care plans incorporating a wellness approach.
o Coordinating appropriate care and services to ensure best possible outcomes for consumers.
o Regular monitoring and review of services to ensure goals are being met.
* Contributing to a multidisciplinary team approach.
* In partnership with consumer, manage the HCP budget allocation.
* Accurate and timely documentation and maintenance of all consumer records.
* Conduct clinical assessments for consumers using standardised tools, within program specific requirements.
* Provide other specific services (including direct care) for consumers within your scope of practice as a clinical health professional.

Minimum Requirements
Registered or Enrolled Nurse
* Experience working in an aged, health or community setting.
* Current motor vehicle driver's licence
* Two (2) professional referee checks - worked with candidate recently
* National Police Certificate obtained within the last three (3) months
* Working with Vulnerable People card.
Highly desirable
Strong interpersonal and verbal/written communication skills enabling effective interaction with consumers and their families, team members, external health professionals and key stakeholders.
* Comprehensive understanding of community-based service provision.
* Demonstrated computer literacy with experience in word processing, calendar management software (Outlook) and data base applications.
* An understanding of risk management principles, workplace health and safety and continuous quality improvement processes and their application in the workplace.
* The ability to work autonomously and as a member of a multidisciplinary team.
