Job Title: Registered Nurse - Community Nursing
About the Role
We are seeking a passionate and enthusiastic Registered Nurse to join our friendly community nursing team in the Hunter Valley. As a community nurse, you will play a vital role in delivering person-centred care to clients in their homes and within our nurse-led clinics.
Your Key Responsibilities
* Coordinate person-centred care for a diverse caseload, including wound management, CVAD management, and urinary continence management.
* Work primarily Monday to Friday, with occasional weekend shifts as required.
* Provide care in both home and clinic settings, including childhood immunisation clinics, school-based immunisations, and audiometry screening clinics.
* Work within a multidisciplinary team, case managing patients to ensure positive health outcomes.
About You
* You must be a current registered nurse with AHPRA and eligible to drive in NSW.
* You should have recent experience in acute care, community health, or outpatient settings.
* You should possess excellent interpersonal and communication skills.
* Strong wound assessment and management skills are desirable.
* You should be able to work autonomously, manage priorities, and adapt to changing patient needs.
