Ozcare is looking for a new Care Manager Lead - to support the Care Manager team to develop and grow their Home Care Package business within their branch.
Your Role
Reporting to the Branch Manager, you'll be responsible for providing leadership to a dedicated Care Manager team in the planning, managing and leading client care and support across a range of programs. You'll also play a key role in co-ordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care. You will play a pivotal role in ensuring consistency in governance and compliance, while driving service growth.
As a Care Manager Lead you will:
1. Manage and co-ordinate care services to ensure the highest standards of care
2. Develop and implement care plans tailored to individual client needs
3. Liaise with clients, families, and healthcare professionals to ensure comprehensive care
4. Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
5. Ensure compliance with all relevant legislation and organizational policies
6. Monitor and evaluate care services to continuously improve quality and efficiency
7. Monitor the budget of Home Care Packages and ensure clients understand their funding and fees
8. Work closely with the Branch Manager and Care Managers to ensure service growth
9. Provide supervision and support to the Care Manager team

Role Requirements:
15. Demonstrate your experience in case management and client care co-ordination in a Community Aged Care setting
16. Demonstrate you have strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
17. Show the ability to develop staff, foster teamwork and effectively work as a team member whilst providing leadership
18. Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
19. Provide your current First Aid and CPR Certificate
20. Provide a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check) - or apply for these prior to commencing in the role
21. Current National Police Check, prior to commencement
22. Hold a current Queensland driver's licence
23. Provide any tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
