Aged & Disability Support (Community Services & Development) Full time 1 x Aged Care Connector – Full-Time - 38 hours per week Based in Toowoomba Purpose of the Position: The Aged Care Connector reports to the Aged Care Manager. The role will contribute to ensure Goolburri clients receive the highest possible quality of care, by ensuring excellence in all aspects of our practice. Position Requirements: The Aged Care Connector is responsible for providing support such as: Education sessions for older Australians individually or group setting around process of My Aged Care system. Access/referrals to the My Aged Care System for older Australians needing Commonwealth Home Support Program or Home Care Package services. Support older Australians in the process with Regional Assessment Service. Providing necessary, appropriate, easy to understand and accurate information in health management, available services, and resource accessibility. Delaying or reducing the number of hospital admissions, including those for older people living with a chronic condition that can be managed at home or in the community. Strengthening culturally appropriate health pathways to ensure primary care, community and at-home services meet the cultural needs of Aboriginal and Torres Strait Islander people and Non-Indigenous people. Creating meaningful social participation that connects people to family, friends and community. Improve the older Australians ability to coordinate the care they require, between: GPs, Practice Nurses, health care workers, allied health and secondary health professionals. Hospital and Health services. Government and non-government funded community services (including services accessed via My Aged Care). Mandatory Requirements: Cover Letter and Current Resume.
