**About the Role**
As a Clinical Care Coordinator, you will be responsible for the clinical care of care recipients within our facility.
You will work with our leadership team and an experienced residential and care manager in our medium-sized care home in VIC.
You will provide clinical leadership to nursing and care staff, mentoring and inspiring them to deliver outstanding care while supporting their ongoing development and career progression.
You will use your previous experience to oversee the clinical governance of the facility and optimize AN-ACC funding.
You will coordinate your team and ensure safe, clear, and effective communication across the multidisciplinary team.
**Key Responsibilities**
* Provide clinical guidance to residents and care staff.
* Undertake resident assessments and case conferencing in line with standards.
* Accurately complete, review, and update relevant documentation.
* Motivate, train, and develop clinical team members.
* Ensure compliance and quality indicators are met.
**Requirements**
* AHPRA Registered Nurse registration with previous leadership experience.
* Up-to-date working knowledge and experience achieving accreditation standards.
* The ability to identify and improve areas of clinical risk and quality guidelines.
