Overview
You will provide client services to people in a South East community who are aged, frail or disabled and who wish to remain living in their own home with some assistance. Limestone Coast Local Health Network (LCLHN) provides care under a Consumer Directed Care model for the provision of aged care, disability and support services. This allows consumers to maintain greater control over their lives by allowing them to make choices about the services they receive and who delivers those services. As a Community Support Worker, you will provide services including personal care, cleaning, meal preparation, transport and social support to clients. You will work closely and collaboratively with the consumer, their family and other health professionals to provide quality outcomes and maintain consumer independence where possible. As part of your role, you may also work within a Day Centre Activities centre. Limestone Coast Local Health Network supports its staff to further develop their skills across a variety of areas. Ongoing education is encouraged, and enables staff to provide quality and responsive services to consumers.
Holding a current driver’s licence and with access to your own transport, you will need to be flexible, conscientious and empathetic and be able to work autonomously in the community. Demonstrating the ability to perform under limited direction, you will work well within a team environment and be able to communicate effectively with people from a diverse range of backgrounds. Along with a commitment to maintaining client confidentiality, you will display empathy towards the elderly and the disabled, and be willing to develop a positive rapport with clients and their families.
Appointment will be subject to a satisfactory Criminal History Check and appropriate immunisation requirements.
