Your New Role
1. Undertake individual Occupational Therapy assessment, intervention, goal-directed care planning and review of clients who may be experiencing ageing, disability or have or are at risk of developing chronic disease, over the age of 18 years.
2. Enable client to maintain their highest level of independence and function and support them to live health, independent, safe, socially connected and valued lives in our community.
3. Provide client centred, goal directed model of care working collaboratively with client and their supports to achieve measurable outcomes.
4. Provide Occupational Therapy services to clients on Home Care Packages and to NDIS participants as well as block funded State and Commonwealth Government programs (i.e. CHSP, HACC)
5. Provide Occupational Therapy component for multidisciplinary group programs as required i.e. Cardiac and Pulmonary rehab as well as Falls Prevention.

Skills and Experience
13. Bachelor of Occupational Therapy or equivalent tertiary qualification
14. AHPRA registration
15. Australian citizenship or Permanent Residency
16. Demonstrated clinical experience and knowledge in Occupational Therapy, home assessment and intervention, functional assessments, Supported Independent Living assessments and Specialist Disability Accommodation Assessments for NDIA Participants.
