The program plays an integral role within Victoria’s health care system, supporting people to maximise their health, independence and functioning, and minimise long-term care needs. The Home, Acute and Community program oversees a diverse group of services promoting effective seamless care, from acute care back into the community. These services are provided in people’s homes, community centres and in hospital when indicated. Our Acute and Integrated Care services operate from our patients’ homes, community services and our inpatient wards at Alfred, Caulfield and Sandringham Hospitals. Working closely with our Emergency Departments, acute specialty units and community partners, we aim to provide high quality acute care, coordinated by an expert interdisciplinary team to meet needs arising from complex acute and chronic disease, in a way that meets our patients’ expressed needs. Our Acute services also play an important role in promoting self-management of chronic conditions and prevention of deterioration through integrated models with primary care. About the Role General Medicine Hospital in the Home (Gen Med HITH) is seeking expressions of interest from experienced nurses for the role of nurse clinician to fill a one year maternity leave vacancy. This is an exciting opportunity to expand your knowledge, and experience caring for complex patients with acute conditions in the community setting. The service cares for patients that would normally be cared for in a hospital bed. Instead they receive their care in the comfort of their home, with the support of nurses, allied health, and medical team members. The nurse clinician is a key role in the multidisciplinary team, ensuring that patient care is holistic and that patients have robust follow up plans following their acute therapy. Gen Med HITH works closely with the acute inpatient wards and the emergency departments, and collaborates closely with the Hospital Admission Risk Program (HARP) and other community-based programs. Duties Systematically assess the patient, analyse data and identify patient’s holistic needs, ensuring the patient is engaged in all decision-making processes related to their care and personal goals. Use knowledge, skills and problem-solving approach to assess, plan and evaluate the effectiveness and appropriateness of the patients’ plan of care. Accept responsibility for own continuing professional development by setting professional goals based on your interests, relevance to the service, and your assessment learning needs. Maintain own skills and knowledge through a self-directed learning process, and by accessing available resources. Initiate and participate in quality improvement to promote quality patient care within the department. Actively manage the department resources to ensure maximum efficiency, including diligent monitoring of patient equipment and consumable requirements. Skills & Experience Essential Registered as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA. Experience within the acute and or ambulatory/community setting desirable.
