Join us in shaping the future of healthcare in Regional Queensland.Your OpportunityTo deliver high level Social Work clinical services and professional expertise within the Integrated Care Services to patients and their families of the Fraser Coast to optimise their psychosocial outcomes and to achieve maximum benefits within the community setting.Work autonomously within the interdisciplinary team to manage, coordinate and provide optimal, responsive flexible Social Work services.The position will focus on chronic health management and hospital avoidance by utilising a range of community supports.What we're looking for:Clinical Expertise:Demonstrated high level expertise and skill in assessment and intervention relevant to Integrated Care Services and Community Health Setting.Demonstrated high level knowledge of Social Work practice theories, social legislation and policies.Demonstrated extensive knowledge of medical, surgical and chronic disease conditions which can impact on a person's functional ability in the community.Communication:Demonstrated high level of communication, interpersonal and consultative skills required to work cooperatively and effectively with members of other disciplines and to liaise with relevant community service providers.Clinical Ability:Demonstrated ability to assess and respond in a professional manner to complex clinical issues for consumers of the WBHHS where intervention is primarily crisis orientated.Leadership:Demonstrate ability to contribute to, initiate, plan and evaluate local service improvement initiatives.Demonstrated ability to promote professional standards, trust, open communication and self-responsibility.Demonstrated ability to provide education, supervision and support to less experienced Social Workers and students.Preferred Qualifications:Mandatory possession of a recognised degree in Social Work, and eligibility for membership with the Australian Association of Social Workers (AASW).
Eligibility is dependent on the candidate having completed a University level Social Work degree accredited by the AASW and that the candidate does not have any other outstanding professional competency or ethical issues or complaints known to the AASW which may preclude membership.Overseas qualified Social Work candidates require a "certification of eligibility" letter from the AASW to confirm their eligibility for membership.
Overseas qualified applicants should include such evidence with their application to be considered for short-listing in the selection process.Appointment to this position requires proof of qualification and if applicable registration or membership with the appropriate registration authority or association.
