Dementia Support Worker
Wallsend Campus/ Awabakal Country
1. The Dementia Advisory Service is a team of two (2) staff located on the Wallsend Campus.
2. DAS is part of the CACS GNS Aged Care Services. Other teams within the Aged Care Services include Aged Care Assessment Team (ACAT), Regional Assessment Service (RAS) Community Dementia Nurses, Neuropsychology Service and Hunter Carer Support Officer.
About the job:
3. In this role you will support and provide information to carers of people living with dementia and people with dementia. Provide dementia education to various stakeholders.
4. This position helps promote local community awareness of dementia, available services etc.
5. You will lead local dementia carer support groups.
6. Upon commencement of this role you will be provided with orientation and will receive ongoing support from CNC Dementia and CNC Aged Care.

Requirements:
Our successful candidate will possess the following:
13. Registered Nurse, Occupational Therapist, Social Worker, or Physiotherapist with appropriate qualifications and/or AHPRA registration and/or membership to relevant professional body in NSW.
14. Eligibility to drive in NSW.
15. Experience working with people living with dementia and their carers.
16. Ability to prioritise work load within a small team.
17. Demonstrate excellent written and verbal communication skills presentation skills, working with diverse groups.
18. Demonstrate professionalism in an empathic and caring manner.
19. Understanding of dementia and ability to provide support for people with dementia, their families and carers.
20. Understanding and provision of person-centred care in collaboration with clients, carers and service providers to achieve identified outcomes.
