The Primary and Community Health Social Work Department have opportunities for you to work with children and their families across the following teams: About The Opportunity The Social Work Team is responsible for providing responsive psychosocial interventions and support to families across SWSLHD. The Social Work Team supports families from a strengths-based model, providing holistic care within trauma informed, culturally safe interventions. Social Work ensures psychosocial supports and interventions are established in collaboration with the family and are adaptive to reflect their changing needs. The Social Work role is diverse as it works with various internal and external stakeholders to provide service collaboration, advocacy, case management and consultation. The roles require an understanding of diverse family structures and various Health Service access points. You will be working with families in collaboration with Multi-Disciplinary teams, completing face to face home visiting, participating in case reviews and contributing to the holistic service provision of families in SWSLHD. We are looking for a Social Worker who is passionate about making a difference in the lives of children & their families and who have experience in the areas of mental health, domestic violence, and child protection. A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW). Demonstrated experience in a health related field and ability to provide a range of Social Work interventions. Demonstrated experience prioritising work and managing stressful situations. Proven ability to work independently and collaboratively within a multi-disciplinary team. Demonstrated high level communication skills (written and verbal) and computer skills. Evidence of commitment to clinical supervision and professional learning. Demonstrated understanding of, and commitment to quality processes, and the application of evidenced based practice within social work.
