About the Role As an Occupational Therapist with the Care of the Older Person and Community Transition (CO-ACT) service at Hampstead Rehabilitation Centre, you will play a vital role in supporting patients through specialised assessment and intervention. Working independently, you’ll apply advanced skills to manage complex, high-acuity caseloads, ensuring each patient receives high-quality, personalised care aimed at enhancing their independence and overall well-being. In this dynamic role, you’ll collaborate closely with a multidisciplinary team, contributing to effective and efficient patient care. You’ll also have opportunities to engage in quality improvement and research initiatives, helping to drive innovation and enhance service delivery standards.
