About the Role As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Rockhampton and surrounding local areas.
Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit evolving needs.
You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families.
Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems.
You will deliver exceptional service to ensure our clients and their families feel valued and supported, while also fostering the growth of our aged care services.
This role is based in our Rockhampton office, with some travel required to clients' homes across the region, including Yeppoon, as required.
We're looking for a candidate with Aged Care experience who can hit the ground running.

Key Responsibilities Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholdersEnsure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visitsMonitoring the clinical risks and needs of a client, ensuring referral pathways are established for the safety of our clientsCollaborate effectively with internal stakeholders to ensure clients' needs are met including our Rostering, Practice and Workforce teamsDevelop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the communitySkills & Experience Demonstrated Aged Care or other clinical case management experience with strong administration skills, and ability to work with individuals, families and/or stakeholdersUnderstanding of Home Care Packages, Commonwealth Home Support Program, Aged Care services and My Aged Care StandardsClinical experience and knowledge will be highly regardedHigh level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systemsAbility to identify and implement risk mitigation strategiesHigh level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills.Ability to multi-task and work autonomously in a fast paced environmentA current unrestricted Australian Driver Licence, reliable vehicle and a willingness to travelSuccessful candidates will be required to clear probity checks including National Criminal History Record Check and complete an Aged Care Statutory Declaration.
