You will work collaboratively to improve the quality of clients care through facilitation the efficient use of resources thereby enhancing quality, cost-effective outcomes. At integratedliving, our Clinical Case Managers work collaboratively with other health professionals to assess, plan and implement restorative or reablement activities for our clients. The focus of this role is to enable prioritisation and individualisation of health management through promotion of health literacy and empowerment of client and their families/carers through coaching, education and use of assistive technologies, to achieve and maintain optimum level of health and well-being. You will be visiting our clients in their homes to assess, plan, implement and coordinate their Short-Term Restorative Care program in Canberra. Enjoy working from home with remote supervision provided You will work within Aged Care Standards. Partner with clients to understand their needs, develop personalised plans, and provide ongoing support throughout the 8-week program Empower and support clients to achieve their optimal levels of functioning independence and self-care You will educate clients, carers and their families around the areas of wellness and reablement including ongoing Chronic Disease Management Facilitate group workshops and support sessions focused on promoting healthy lifestyle and self-management skills, tailored to the 8-week program structure Coordinate care with multiple disciplines including, GPs, specialists, and our Allied Health team to ensure a holistic approach to client care You will enjoy working independently to promote the service and to seek referral of new clients to ensure full occupancy of the program Building and maintain relationships with local health professionals and services to ensure a high quality of care is maintained throughout the program What you need to be considered for the role? Degree qualification in nursing or allied health Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA) 3+ years of clinical or health management experience in Nursing or Allied Health Experience within a health or community aged care setting Ongoing commitment to Continued Professional Development and Education Demonstrated understanding of community aged care guidelines and the principles of primary health care and health promotion Demonstrated understanding of evidenced based care Ability to conduct Clinical Case Management and Health assessments Strong interpersonal skills, problem solving, analytical and reporting skills Ability to work as part of a wider team and autonomously in a remote model of working Demonstrate excellent time management skills to prioritise client needs and manage a busy schedule Strong IT and administration skills and demonstrated competence in delivering services via virtual health technologies and a willingness to learn and adopt new technologies Contribute to a positive and supportive team environment Current driver's license and ability to travel within the region on a regular basis If you have a passion for supporting the health and well-being of older adults, we would love to hear from you!
