About the team: The Self-Management Support service sits within the Community Health Programs and operates from GV Health's Corio Street Campus. We are a friendly and supportive multidisciplinary team who work collaboratively to provide high-quality care to consumers accessing our services. About the role: We are seeking an experienced and motivated clinician to join the Self-Management Support team as a Health Coach. In this role you will be responsible for supporting consumers by providing health coaching for early intervention in chronic disease with a particular focus on Quit nicotine coaching and individual or group programs which address self-management principles such as nutrition, physical activity, medication compliance, emotional health and connection to support services. You will have an opportunity to develop new group programs to meet identified consumer needs/goals and will also actively engage with the community to raise awareness of chronic disease self-management. We are looking for: Current registration with AHPRA as a Registered Nurse with a minimum of 3 years clinical experience. Additional post-graduate qualifications in Chronic Disease, Self-Management Support, Health Promotion or Population Health would be desirable. Excellent communication, decision making, advocacy and negotiation skills. Knowledge, skills and experience in facilitating self-management, self-help or health education/coaching programs. A thorough understanding of the health service system, referral pathways and networks, including primary health, welfare, mental health, aged care, drug and alcohol and community care services.
