You will develop, implement, review, and maintain comprehensive patient care plans, collaborate closely with the multidisciplinary healthcare team to ensure coordinated patient management, and conduct thorough patient assessments to facilitate outpatient and inpatient treatments.Liaise with physicians, other health professionals, and support staff to coordinate patient management and optimize treatment outcomes.Conduct thorough assessments of patients' physical, psychological, and social needs, identifying any changes or concerns and initiating appropriate interventions.Coordinate outpatient and inpatient treatments, including diagnostic procedures, therapies, medications, and follow-up care, in alignment with prescribed treatment plans.Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in electronic medical records.ABOUT THE HEALTH CENTRERobertson Health Centre is located on Robertson Barracks in Holtze, 15km east of the city of Darwin.
