Position Overview
As an Intake Officer with a nursing background, you will be instrumental in the onboarding process, meeting clients face-to-face to assess their needs and establish care plans. You’ll serve as a trusted resource, guiding new clients and their families through the initial intake process and ensuring they receive the support and information they need for their unique journey.
Key Responsibilities
* Conduct home visits with potential and new clients, performing initial assessments and developing individualized care plans.
* Provide thorough, compassionate customer service, helping clients navigate care options and services.
* Respond to inquiries via phone, email, and in-person visits, offering comprehensive information about our services.
* Ensure accurate and prompt documentation in our client management system.
* Triage and coordinate client intake tasks, liaising with support teams to align services with client goals.
* Build relationships with clients, their families, and community resources, promoting H1 Healthcare and Jim’s Home Care’s services.
* Monitor client satisfaction, engaging in quality improvement initiatives.
* Support clients from diverse backgrounds, engaging with our cultural support services as needed.
Essential Criteria
* Nursing qualification and experience in community-based care, aged care, or disability services.
* Knowledge of HCP and NDIS guidelines, compliance standards, and funding policies.
* Proven experience with intake assessments and care planning.
* Strong interpersonal skills, attention to detail, and the ability to work independently.
* Current Police Clearance, Working with Children Check, valid driver’s license, and an insured vehicle for client visits.
