The Role The Nursing and Midwifery – Our Strategy for Transforming Person Centred Cultures 2024-2029 guides our profession in the development of effective person-centred practice and cultures within SESLHD. We advocate for shared decision-making, collaborative relationships, compassionate and person-centred leadership, and supporting innovative practices. The position supports clients using advanced nursing skills and culturally responsive care to enhance the health outcomes of Aboriginal children and their families. It coordinates service navigation for Level 2 and 3 families and liaises with partnership organisations and is a resource with both internal and external stakeholders. It supports quality improvement and maintains a culture of safety and governance in the C&FH service. The role collaborates with other Statewide services Building Stronger Foundations, New Directions and local Aboriginal community organisations. As a Clinical Nurse Specialist Grade 2 in SESLHD you are recognised as a healthcare professional employing your clinical skills and critical thinking in delivering high quality patient care. You exercise extended autonomy of decision making and professional knowledge and judgement in providing complex care requiring advanced clinical skills. As a Clinical Nurse Specialist Grade 2 you lead the development of the nursing specialty clinical practice and service delivery in the service. You are an integral part of the interdisciplinary team in the provision of person-centred care; and lead, support and supervise other nurses. You will strive to ensure that the people who receive healthcare experience exceptional care that is person-centred, safe, high quality and compassionate. Some of the key functions of your role will be: Deliver optimal outcomes for Aboriginal families and children 0-5 years requiring additional support by promoting best practice, evaluation of care outcomes and provision of culturally appropriate care. Develop collaborative working relationships with CMC Complex Care for families identified in pregnancy and promote engagement throughout early childhood. Provide culturally responsive care navigation for Aboriginal families across the health system. Facilitate family engagement with local services and transitions between service providers. Provide leadership, management; coordination and monitor families and children 0-5 years as they progress through their health care journey. Selection Criteria Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife, a current NSW Driver’s License and willingness to travel as required in role. Relevant post-registration qualifications and at least 3 years’ recent experience working in the Child & Family Health clinical area with Priority Populations, or such other qualifications or equivalent experience deemed appropriate. Demonstrated knowledge and understanding of the local Aboriginal community, including an awareness of the issues that impact on the health and wellbeing of Aboriginal people and the provision of culturally appropriate care. Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans. Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations. Demonstrated skills in the development and facilitation of patient and carer education; and ability to mentor and coach nursing staff to enhance professional development. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others and competence in the use of health-related information technology. Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
