CCQ’s diverse region covers 161,108 km2 from Sunshine Coast to Rockhampton and west to Emerald. The population in the region is just under 1M people with around 68% living regionally or remotely and more than 28% living in the most disadvantaged quintile. CCQ collaborates with the community to provide exceptional primary healthcare services to reduce health inequalities for the region. CCQ has seven key priorities for health improvement: mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, population health, health workforce, digital health and older person’s health. About the role: The role reports to the Senior Manager Regional Programs (Sunshine Coast) and is part of a team of four Primary Health Coordinators. You will be responsible for building and maintaining strong relationships with general practices and other primary health providers. Acting as a key liaison, the position involves influencing high-performing service delivery and improving outcomes through data-driven quality improvement A core aspect of the role includes contributing to the implementation of localised health initiatives, increasing community access to effective and efficient healthcare services, and ensuring these align with organisational priorities. The successful candidate will engage with stakeholders in a territory management model, supporting primary care providers, promoting organisational objectives and fostering partnerships to achieve shared goals. By understanding regional health needs, the position supports evidence-based planning, delivery, and evaluation of health programs. The Primary Health Coordinator will support a variety of quality improvement projects, develop territory plans, overseeing implementation, data collection, activity scheduling, and risk management while reporting outcomes. Additionally, the role plays a pivotal part in advancing health outcomes for Aboriginal and Torres Strait Islander communities and enhancing the capacity of mainstream providers to deliver culturally safe healthcare services. About you: Applicants should posses tertiary qualifications in a health or other relevant discipline, coupled with over five years of professional experience in the healthcare sector. The ideal candidate will: Demonstrate expertise in delivering measurable results through evidence-based health service engagement, implementing change management processes, and overcoming barriers to collaboration. Possess a deep understanding of the primary healthcare system, general practice operations, and relevant government policies is essential. Highlight strong skills in project management, stakeholder engagement, and the ability to use digital systems to enhance care integration are also required.
