Social Worker Grade 2- Rural Allied HealthEmployment Type: Part Time Fixed-Term position until end of June 2025 (maternity leave replacement)Location: SheppartonAbout the Hospital:We pride ourselves on delivering person-centred care.We aim to enhance patient experience through improved service access, developing partnerships, meeting growth in demand, implementing innovative service models and ensuring workforce flexibility.About the role:Join a well-renowned supportive workplace culture with an opportunity to work alongside experienced Allied Health Professionals.In this role, you will work autonomously, undertake a varied and diverse workload, and provide interventions in predominately home-based settings, making a true difference to our client's independence and well-being.There is no nursing home work.Our multidisciplinary team includes Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work, and Speech Pathology services, supported by Allied Health Assistants.A comprehensive orientation program is available.The Social Worker will provide person-centred care with an opportunity to:Travel across the Goulburn Valley region working in home-based settings.Work with clients, carers, and families to provide support, counselling, information, advocacy advice, and referral with a focus on informed decision-making and empowerment solutions to promote independence at home and in the community.Develop goals and care plans to assist clients to remain at home and active in their local community.Advise on strategies and areas of assistance to support clients to remain living independently at home and within their local communities.Work with a diverse range of clients and health conditions.Participate in student training programs. We are looking for: Bachelor of Social Work or Master of Social Work (qualifying) and a member of the AASW.Broad knowledge in the planning, implementation, and evaluation of current Social Work practice within health care settings including home-based and community settings.Experience in the delivery of Social Work services to clients with complex presentations, inclusive of mental health problems, complex health conditions, cultural and diverse backgrounds, and social disadvantage.Experience in providing services within a self-management framework and active service model/reablement approach to care delivery.Experience working with frail older people, people with disabilities and their carers, and people living in rural communities.A clear/satisfactory National Disability Insurance Scheme Worker Screening (NDIS) Check upon commencement.
