About the Role:
We're seeking an experienced Grade 2 Occupational Therapist to join our HARP Disease Management Team (HARP DMT) on a permanent part-time basis. Our team is a well-established, supportive, and multidisciplinary group, dedicated to delivering high-quality outcomes for our clients.
HARP DMT focuses on enhancing client well-being and reducing hospital readmissions for individuals with chronic cardiac or respiratory conditions. This community-based program emphasises person-centred care, offering intensive care coordination for those in need.
In this role, you'll work autonomously, managing a varied and diverse caseload, primarily providing interventions in home-based settings. You'll have the opportunity to make a meaningful impact on our clients' independence and overall well-being.
This position can be combined with other roles within GV Health, offering the potential for increased or full-time hours.
The successful applicant will have:
* Approved tertiary qualification in Occupational Therapy and Current registration with the Australian Health Practitioner Regulation Agency
* Sound knowledge of person centred care and experience in the planning, implementation and evaluation of Occupational therapy services within the home and community settings
* Demonstrated knowledge and experience working with people who have chronic illness
* Excellent interpersonal, written & verbal communication skills including problem solving ability & computer literacy
