With 110 residents across the sites and 200 staff to support them, we strive to be like a warm hotel with quality health care where our residents are supported to celebrate life. Our Swansea facility also offers sub-acute and emergency services as well as residential care.

About the role
The purpose of this role is to provide high-quality person-centred care to residential and home-based aged care clients. Our Vision is to support our communities to celebrate life at every stage. We do this by working with our rural communities to provide responsive and innovative health and aged care services to enable our local people to enjoy quality care in their home community.
Key responsibilities
The key focus is providing high-quality person-centred care support to consumers, within scope of practice, working collaboratively with all stakeholders, including colleagues, residents, their families, and the broader care team, to deliver holistic outcomes. Daily activities include:
• Providing personal care to assist residents to maintain independence and dignity related to activities of daily living and lifestyle
• Working within a multidisciplinary team inclusive of other ECAs, Hotel Services, Lifestyle, Nursing and other staff to achieve the best care outcomes
• Completing all documentation in an accurate and timely manner, and assisting with assessments and reviews as necessary
• Demonstrate consideration, understanding and respect for clients and their families at all times in all interactions.
About You.
To be successful in this role you will need:
• Certificate III or IV in Individual Support (or equivalent).
• Experience in an Extended Care Assistant or Personal Care Worker role.
• Ability to work across all shifts.
• A current Police Check (not more than 12 months old)
• COVID and Flu vaccinations
