The Care Finder Role:
As a Care Finder, you will:
* Provide specialized, intensive assistance to help people navigate and access aged care services.
* Break down barriers for recipients, connecting them with vital community supports.
* Improve integration between health, aged care, and other local systems.
* Engage with older individuals facing significant challenges in accessing aged care services.
* Collaborate and communicate effectively with participants, their families, and relevant stakeholders.
Our Ideal Candidate Will Have:
* Qualifications in Allied Health, Nursing, Community Services, Social Work, or equivalent.
* A solid understanding of the aging process and related life transitions.
* Knowledge of local and regional aged care supports and services.
* A commitment to a person-centred approach that respects individual needs and experiences.
* Well-developed communication and interpersonal skills, with the ability to connect with diverse backgrounds.
* Strong problem-solving abilities and a solutions-focused mindset.
* Proficient administrative skills, including experience with client management systems and databases.
* A current Australian driver’s license and a willingness to travel occasionally for work.
