Aged & Disability Support (Community Services & Development)
STAR Community Services is looking for an NDIS Support Coordinator to join our team in Cleveland, part time 52.5 hrs per F/N. STAR Community Services LTD is a leading not-for-profit organisation providing transport, aged care, and disability support services to the elderly, frail, and those with disabilities. About the Role:
The NDIS Support Coordinator will be responsible for establishing a positive collaborative relationship with the person and their support network and assist the person to identify, link with and coordinate support to link with local communities, build skills, overcome barriers and achieve goals.
You will have the responsibility to ensure the implementation of person's plans and the achievement of their goals. The NDIS Support Coordinator will need to have a strong understanding of the NDIS and the role of an NDIS Support Coordinator, and will be required to support people who have coordination of support in their plans to access supports more effectively.
Key Responsibilities:
1. Assist NDIS participants to build their capacity to understand and coordinate their NDIS plans.
2. Ensure support coordination is carried out in line with the agreed work schedule and claims for payment are lodged in a timely manner.
3. Regularly monitor participant expenditure and provide information regarding rates of expenditure and potential impacts.
2. Service Coordination & Advocacy:
1. Identify, coordinate, and manage a variety of services and providers that meet the participant’s needs.
2. Navigate, influence, and negotiate with both public sector and community-based service systems to ensure the delivery of appropriate supports in accordance with the participant's NDIS plan.
3. Liaise and collaborate with relevant external parties, including NDIA and service providers, to ensure the best outcomes for the participant.
3. Capacity Building & Community Connection:
1. Use local knowledge and sector expertise to create opportunities for NDIS participants, particularly those with lived experience of mental health challenges, to engage with local communities.
2. Support participants and their families to understand and navigate the service system and to build resilience and skills within the participant's network.
4. Expert Advice & Consultation:
1. Provide expert advice to participants and their families regarding changes in the NDIS environment, ensuring they are kept informed of policy and procedural updates.
2. Foster positive relationships with participants and their families to ensure the effective delivery of services in alignment with the participant’s goals and objectives.

About You:
The successful candidate will demonstrate a high level of professionalism, empathy, and dedication to improving the lives of people with disabilities. We’re looking for someone who is passionate about supporting people with disabilities and has the following skills and experience:
Experience in NDIS Support Coordination:
1. Previous experience in NDIS support coordination or a similar role is highly desirable.
2. Strong understanding of NDIS processes, funding models, and plan management.
3. Strong verbal and written communication skills, with the ability to engage effectively with participants, families, service providers, and other stakeholders.
Person-Centred Approach:
1. Ability to develop and implement individualised support plans that reflect the goals and preferences of participants.
Problem-Solving and Negotiation Skills:
1. Ability to navigate complex service systems, negotiate with service providers, and advocate for participants to access the services they need.
Organisational and Time Management Skills:
1. Strong organisational skills and the ability to manage multiple tasks, priorities, and deadlines.
Qualifications:
1. A qualification in Social Work, Allied Health, Disability Services, or a related field is desirable.
