The CNC collaborates with members of the multidisciplinary treatment team and relevant community service providers to coordinate the delivery of patient-oriented, specialised support services across the care continuum.This is a part-time position (4 days/fortnight) Monday to Friday on a rotational day roster excluding public holidays.About You:A passion for Oncology nursing with a keen interest in Breast CancerExceptional organisational and prioritisation skillsCurrent driver's licenseBased at Moorabbin Hospital with ability to travel to other Monash Health sites, as required/rostered.You have demonstrated skills in advanced practice nursing with the ability to work autonomously and as part of a teamA high level of proficiency in integrating contemporary knowledge and evidence about breast cancer to provide a range of supportive care interventionsExpertise to plan, coordinate and provide advanced breast care management across the care continuum to patients with breast cancer.Collaborate with the patient receiving care, their family and/or significant others, other nurses and members of the health care team, to optimise health outcomes for patients with breast cancer.Passionate about educating patients with breast cancer to enable the patient to be active participants in their care and engage in self-management of health-related problemsYou have experience in contributing to continuous improvement and a passion for the advancement of specialist breast cancer nursing care.Able to participate in related professional networksWhat You Need:Breast or oncology qualifications highly desirable or working towards a Graduate Certificate, Graduate Diploma or Masters of Breast/OncologyDemonstrated experience in Women's Health and/or breast disease requiredDriver's licenseIn Return:An enthusiastic, passionate, high performing multidisciplinary team environment delivering excellent patient care.Ongoing professional developmentSupportive learning environmentFor a confidential discussion and to explore the opportunity further, please call Gabrielle Bunton 9928 8620 or Catherine Langford on 9928 8760.Position Description can be found hereApplications close: Monday 18th November 2024#J-18808-Ljbffr
