About the Role Are you passionate about making a difference in the lives of older people living with mental illness? We are seeking a compassionate and dedicated Clinical Nurse or Mental Health Professional to join our dynamic team. In this role, you will work closely with older people to provide specialist mental health care, ensuring they remain engaged in the community and reduce unnecessary transfers to acute care settings. Your expertise will play a vital role in preventing adverse incidents and improving mental health outcomes through collaborative partnerships and high-quality, recovery-focused services. You will coordinate care pathways across the region, deliver direct clinical support, and facilitate access to telepsychiatry services and inpatient admissions as needed. Your role will involve both direct clinical contact and distance consultation, offering guidance and support to ensure the best possible care for older Australians. Key Responsibilities: Provide specialised nursing and mental health care to older people with mental illness Work collaboratively to prevent avoidable hospital admissions Act as a conduit to telepsychiatry services Lead clinical practice and mentor less experienced staff Contribute to the development and refinement of care pathways Minimum Requirements: Proficiency with Microsoft Office and clinical documentation A commitment to high-quality care and professional growth Clinical Nurse (RN/M2C) Classification: Registered Nurse with a postgraduate qualification in Mental Health and at least 3 years' post-registration experience.
