Join us in making a difference!The RoleWe currently have an exciting opportunity for a highly motivated and person-orientated individual to join our organisation as a Social Worker at our Mosman Park Care Centre working permanent Part-Time on a fixed term contract working 45.6 hours per fortnight ending May 2025.In this dynamic role, you will play a crucial role in promoting the psychological safety and well-being of clients/residents and their representatives as they access Amana Living Services. This position provides vital information and support tailored to each individual’s complex care and social circumstances. The Social Worker assists residents and their families in understanding their care needs and navigating the intricacies of care planning, decision-making and provision of community-based services. Additionally, you will collaborate with a multi-disciplinary team to empower clients and their families to achieve their goals effectively.Week 1 & 2Monday: 0800-1615Thursday: 0800-1615Key responsibilities:Undertaking social work assessments, including psychosocial assessments for all assigned clients in consultation with other team membersLiaising with clinical staff/pharmacy and informing them of residents’ planned discharge dates for transport arrangementsCase management may include supporting clients with State Administrative Tribunal (SAT) and guardianship processes.Planning and undertaking social work interventions with clients, in conjunction with their families and the multidisciplinary teamProviding care coordination, counselling, advocacy, mediation, referrals and support for clients when transferring them to either their home environment or a Residential Care FacilityProviding consultation and support to Nursing and Allied Health professionals regarding psychosocial aspects of care for clientsEnsure compliance is maintained in line with Amana Living's policies and procedures, and accreditation standards are sustained.Provide case management and discharge planning services to ensure that the client can either:• remain with the provider as a permanent residential client, or• be discharged home (community) with appropriate supports, or• be discharged to another provider.About youThe successful candidate will demonstrate experience working in a fast-paced role in an organised and adaptable manner.
