About the RoleConsultant Rehabilitation Physician who will provide clinical services to the Monash at Home service. This service provides a bed substitution service for patients residing in our catchment. You will work with a multidisciplinary team comprising of a Consultant Geriatrician's, Advanced Trainee's in geriatric and rehabilitation medicine, HMO's, nursing and allied health clinicians including; physiotherapists, occupational therapists, dieteticians, social workers, speech pathologists and neuropsychologists. You will be responsible for providing patient centred, evidence based care to patients in their own home both face to face and via TeleHealth. Provide clinical leadership through active involvement in patient care Be available to medical colleagues for consultation and liaison as appropriate regarding patient care Contribute to the professional and corporate life in Victoria's largest public health service by participation in appropriate committees and educational platforms. Contribute to ongoing service improvement and clinical and workforce innovation to ensure Monash Health provides high quality accessible Rehabilitation Medicine services within allocated resources and meets the needs of Monash Health patients.
