You will be working at Royal Prince Alfred Hospital. In this position you will:Co-ordinate the patient's process through the hospital system by providing complex case management, facilitate timely access to care and support discharge planning.Recognise deteriorating/unstable patients based on verbal and written clinical handovers (including diagnostic tests, ie bloods, ECGs, etc) and liaise with Patient Flow Unit on the priority and plans of Inter-hospital transfers.Ideal CandidateCurrent nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field.Holds approved postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.Recent Clinical experience in Cardiology in a Tertiary Referral setting.Able to work a Monday to Friday roster.Previous experience in patient flow or at least understanding of patient flow (beneficial not mandatory)Benefits and Perks!
