Key responsibilities: Partner with individual clients and residents to support lifestyle activity choices to meet the needs of the individual, improve socialisation choices and maintain community connections where able. Explore an individual’s interests and partner with them to develop and enhance their skills and lifestyle choices to encourage and support them to consider and plan toward suitable individual and group activities. Evaluate, document and report on individual’s outcomes, successes and adjustments for Leisure and Lifestyle activities, to immediate supervisor to ensure individuals are achieving their specific goal or desired outcome.
