About the role
The Senior Social Worker Level 3 in Aged Care will provide clinical expertise for patients admitted under Aged Care for the Geriatricians and the Aged Care Social Work team. This role will be expected to:
1. Conduct complex psychosocial assessments for admitted patients.
2. Effectively work as a team member within a multidisciplinary team.
3. Conduct effective and timely discharge planning which includes referrals to the other services and organisations.
4. Provide supportive counselling regarding the adjustment to illness and/or disability and the impact of the chronic illness.
5. Understand NCAT and NDIS process.

6. A Bachelor degree in Social Work or equivalent that provides eligibility for full membership of Australian Association of Social Work (AASW) with experience at Level 2 or higher.
7. At least 5 years work experience in the field of Social Work.
8. Ability to participate in the After-Hours roster and present to the hospital within an hour of being called.
9. Recent employment within a hospital or health care settings is preferred.

About working for SLHD
All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course).
