In the role of Occupational Therapist you will focus on: Delivering Comprehensive and Tailored Care : Providing high-quality occupational therapy services, including assessments, therapy, and management, to patients with a range of conditions, ensuring individualized care for developmental, medical, surgical, orthopaedic, and chronic upper limb conditions. Collaborative Multi-Disciplinary Approach : Coordinating seamlessly with a multi-disciplinary team, contributing to team meetings, therapy sessions, and family discussions to ensure holistic care and optimal patient outcomes throughout their rehabilitation process. Ensuring Continuity of Care : Managing patient transitions with care and attention, ensuring smooth referral processes to relevant community agencies, and facilitating discharge planning to promote long-term well-being and recovery for each patient. Building Integrated Support Systems : Fostering strong relationships with families, staff, and local community services to ensure a well-rounded, integrated approach to patient care, creating a seamless transition from hospital care to home, community, and adult services.
