The Role
As our Care Manager, you will work as part of a team of empathetic and caring clinical professionals.
You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance, helping them to live longer, healthier, and happier lives.
Duties and Responsibilities
* Oversee the daily clinical operations of the care home
* Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
* Review clinical incidents, complaints, and changes
* Lead compliance with AN ACC assessments
* Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
* Prepare and conduct clinical governance audits
* Oversee medication management in accordance with resident needs and legislative requirements
* Liaise with allied health, medical practitioners, and other services to provide optimal resident care

What Will You Bring?
* Bachelor of Nursing with current AHPRA is essential
* Post grad qualification in aged care highly regarded
* Experience managing a minimum of 30 beds
* Proven track record in delivering quality clinical care within an aged care or acute setting
* Excellent clinical assessment skills and care planning capabilities
* Highly effective communication, leadership, and relationship management skills
* An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
