Whether you're a skilled clinician, experienced assessor, or a passionate support professional, we invite you to join our dedicated team and help us make a real difference in people's lives.ABOUT THE ROLE The purpose of this role is to undertake holistic personal Comprehensive Home Support assessments for My Aged Care clients. The Comprehensive Home Support assessments are for people who may need entry-level support to help them stay at home. Footprints Community aged care assessors incorporate the Aged Care Assessment Quality framework to deliver high quality assessments, with a focus on client experience. The aim is to work with clients to develop support plans to achieve wellness and healthy ageing goals through linking and referral to the client's chosen support services through the My Aged Care Gateway.KEY ROLE ACCOUNTABILITIES Complete Comprehensive Home Support Assessments using the Integrated Assessment Tool (and other validated tools when required) in line with program guidelines, timeframes and quality requirements using the standardised Aged Care Assessment Quality Framework.Complete the required volume of aged care assessments as per contractual and reporting requirements.In completing the Aged Care Assessment, follow the principles in the My Aged Care Assessment Manual.Conduct reviews for re-assessment as required.Take a Wellness and Reablement approach to Aged Care Assessments.Facilitate clients' progress towards achieving increased stability and improved quality of life by assisting aged clients, according to individual support/service plans, to access relevant commonwealth aged care services and state-based health and community support services, leisure and recreational activities, medical and allied health services.Ensure clients are connected to relevant service providers and ensure follow up with clients and service providers, where appropriate, to ensure they are receiving the required support and care.Liaise with employees, agencies, service providers, care providers or others on behalf of the client where required.Respond to client enquiries, issues, and concerns in a timely manner.Work as part of Footprints Community's Aged Care Assessment Team to coordinate client referrals and link with appropriate service providers.Support the delivery of coordinated and transparent services through the centralised client record and My Aged Care Gateway.Contribute to the ongoing development and evaluation of program practice, ensuring continuous quality improvement and compliance with the Aged Care Assessment Quality Framework.Working remotely within a region where you will be required to travel long distances between clients.Hybrid working arrangements will be required (in office and work from home) at the discretion of the team leader and manager in accordance with Footprints Working from Home Procedure.General Follow all organisational policies and procedures.Participate in monthly professional supervision sessions, and ongoing learning and development activities.Promote and maintain a safe and healthy workplace, ensuring adherence to WHS policies and procedures.Work within the vision, mission, and values of Footprints.ABOUT YOU Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with eligibility for AASW membership.Demonstrated knowledge and/or experience in working with people who are ageing and their families/carers, people experiencing mental illness, people with disabilities, people who are homeless or residing in marginalized housing and who may be at risk of homelessness, and people who are financially and socially disadvantaged.Demonstrated understanding/experience of the provision of community-based support services to the aged which reflect a wellness approach with a focus on reablement for clients, including assessment, care planning, monitoring, and evaluation processes.Demonstrated knowledge and experience in community networking and working collaboratively with other service providers to plan and provide consistent, responsive, and appropriate client-focused services.Well-developed written and verbal communication skills and computer literacy.Evidence of annual flu vaccination and up-to-date COVID-19 vaccinations, in accordance with Department of Health and Aged Care.National NDIS Worker Screening Check or current Yellow Card.Experience in assessment and goal-oriented support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services.Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach.Local knowledge and/or the ability to network with community aged care service providers and stakeholders within your region.Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite).Previous experience performing in a KPI environment to ensure good quality client outcomes.Current First Aid Certificate and CPR.W HY Footprints?
