These multi-disciplinary services are provided in the centre, community, home or via Telehealth. Following assessment, the work will largely involve equipment prescription, home modifications, and referral and liaison with other services to increase or maintain independence, and reduce risk in the home. The majority of the clients will be over 65 and referred via My Aged Care under the Commonwealth Home Support Program.The RoleThe Occupational Therapy position will sit within the Adult Health team and you will:Provide comprehensive assessments, recommendations and OT services to clients living at homeDevelop and implement person-centred care plansWork within a multi-disciplinary team to ensure a holistic approach to client supportLiaise with relevant agencies to coordinate, facilitate and communicate client treatment and outcomes.
