The Role
A wonderful opportunity has opened up for an experienced Registered Nurse or Enrolled Nurse to provide high-quality care coordination for our Home and Community Services division. This role is based in Tamworth, but we're also open to candidates in Gunnedah who are willing to travel to Tamworth and surrounding towns. The position is flexible and can be either part-time or full-time, depending on your preference.
In this role, you will be responsible for delivering holistic nursing care to our clients. This includes managing a caseload of home care package clients, ensuring their needs are individually assessed and met in line with McLean Care’s values and policies.
We are looking for someone with extensive clinical knowledge, previous case management experience, and a passion for enhancing the lives of our clients. As a Care Coordinator, you will also use your exceptional people skills to provide leadership, support, and guidance to other team members. This is a Monday-to-Friday position.
About You
This role will report to the Community Program Manager. To succeed, you will bring:
* Comprehensive clinical knowledge, particularly regarding age-related health issues
* Previous care planning and clinical assessment experience
* Engaging leadership skills and the ability to support staff to deliver quality outcomes for our clients
* Strong communication, interpersonal, and negotiation skills
* Ability to assist clients to formulate a care plan within their budget and manage client expectations empathically
* Current APHRA Registration as a Registered Nurse or Enrolled Nurse
* Current First Aid and CPR certifications
* An unrestricted driver’s license
* Knowledge and understanding of Consumer Directed Care packages will be highly regarded.
