The Credentialled Diabetes Educator will be a valued member of the Albury Wodonga Community Health Diabetes Service. The purpose of the role is to provide diabetes education, assessment and intervention for patients and their families within the primary and community health service of AWH. To provide expert level consultation to, and integrated care planning with, internal and external service providers. To also participate in diabetes education service planning and development for continual service improvement that is aligned with local population health needs and service context.
