Seeking a Clinical Nurse Specialist - Renal Home Therapies, as a clinical expert in the area of practice who utilises specialist nursing knowledge, skills and experience to assess, coordinate and review patients eligible for home-based renal replacement therapies (Home Haemodialysis and/or Peritoneal Dialysis).
The Team:
The PD Nurse will work alongside the Consultant Nephrologist, Nurse Unit Manager, Renal nursing staff, Chronic Kidney Disease CNC, Vascular Surgical Team, Specialist Clinic staff, Radiology team and other multidisciplinary team members at the Launceston General Hospital and Royal Hobart Hospital. This service will support patients attending the Renal Unit facilities and satellite units of the northern region who are undergoing dialysis or about to start dialysis.
The Role:
* Lead case management in the clinical management and ongoing care for patients undertaking renal replacement home therapies and involve patients and their families/carers in the planning and implementation of care.
* Provide leadership in the review, development and evaluation of policies, procedures, protocols and best practice related to home-based renal replacement therapies and related patient/client care.
* In collaboration with members of the multidisciplinary team plan, implement, evaluate, and support complex case models and interact effectively with all stakeholders to facilitate the provision of optimum patient/client care.
Details of appointment:
Permanent part time, day worker position, working up to 48 hours per fortnight, commencing as soon as we can onboard you.

Salary: $106,570 to $112,440 pro rata per annum. Our Employer 11.5% superannuation contribution is on top of this amount.

Successful applicants will be required to meet the essential criteria:
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
